Welcome to GLOBAL MED
These Terms of Service ("Terms") govern your use of GLOBAL MED's medical services, including our bespoke medical concierge, primary care, IV therapy, ketamine therapy, and related healthcare services.
Please read these Terms carefully before using our services. By booking an appointment, subscribing to our concierge membership, or receiving any medical services from GLOBAL MED, you acknowledge that you have read, understood, and agree to be bound by these Terms.
Acceptance of Terms
By accessing or using GLOBAL MED's services, you agree to be bound by these Terms of Service and all applicable laws and regulations. If you do not agree with any part of these terms, you may not use our services.
These terms constitute a legally binding agreement between you (the patient or authorized representative) and GLOBAL MED. We reserve the right to modify these terms at any time, and your continued use of our services constitutes acceptance of any changes.
Patient Responsibilities
Accurate Information
You agree to provide accurate, complete, and current information about your medical history, current health conditions, medications, and insurance coverage. Failure to provide accurate information may compromise the quality of care you receive.
Compliance with Treatment
You are responsible for following prescribed treatment plans, taking medications as directed, and attending scheduled appointments. If you have concerns about your treatment plan, please discuss them with your healthcare provider.
Communication
You agree to maintain open communication with your healthcare team and promptly notify us of any changes in your health status, contact information, or insurance coverage.
Respectful Conduct
You agree to treat our staff and facilities with respect. We reserve the right to terminate services for patients who engage in abusive, threatening, or inappropriate behavior.
Payment and Billing
Service Fees
Fees for our concierge medical services, consultations, procedures, and treatments will be clearly communicated to you prior to service delivery. Payment is due at the time of service unless other arrangements have been made.
Insurance
While we may accept certain insurance plans, you are ultimately responsible for understanding your coverage and any out-of-pocket costs. We recommend contacting your insurance provider to verify coverage before receiving services.
Membership Fees
If you subscribe to our concierge membership program, fees are charged on a recurring basis according to the plan you selected. Membership fees are non-refundable unless otherwise stated.
Late Payments
Unpaid balances may be subject to late fees and may be referred to a collection agency. Continued failure to pay may result in suspension or termination of services.
Appointments and Cancellations
Scheduling
Appointments can be scheduled through our patient portal, by phone, or via email. We will make every effort to accommodate your preferred appointment times.
Cancellation Policy
We require at least 24 hours notice for appointment cancellations or rescheduling. Late cancellations or no-shows may result in a cancellation fee.
Emergency Services
For life-threatening emergencies, call 911 immediately. Our concierge services are not a substitute for emergency medical care.
Limitation of Liability
Medical Advice
While we strive to provide the highest quality medical care, we cannot guarantee specific outcomes. Medical advice and treatment are based on the information available at the time and professional medical judgment.
Third-Party Services
We may refer you to specialists, laboratories, hospitals, or other healthcare providers. We are not responsible for the services, advice, or actions of third-party providers.
Website and Technology
We strive to maintain accurate and up-to-date information on our website and patient portal, but we do not warrant that the services will be uninterrupted, secure, or error-free. You use our digital services at your own risk.
Limitation of Damages
To the maximum extent permitted by law, GLOBAL MED shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of our services.
Dispute Resolution
Informal Resolution
If you have a complaint or dispute, we encourage you to contact us first to seek an informal resolution. Many issues can be resolved through direct communication.
Arbitration
Any disputes that cannot be resolved informally may be subject to binding arbitration in accordance with the rules of the American Arbitration Association. You agree to waive your right to a jury trial.
Governing Law
These Terms of Service are governed by the laws of the State of Florida, without regard to its conflict of law provisions.
Additional Provisions
Telemedicine Services
Our telemedicine services are subject to additional terms and conditions. By using telemedicine services, you acknowledge that virtual consultations have limitations and may not be appropriate for all medical conditions.
Intellectual Property
All content on our website, including text, graphics, logos, and software, is the property of GLOBAL MED and protected by copyright and trademark laws. You may not reproduce, distribute, or create derivative works without our written permission.
Severability
If any provision of these Terms is found to be unenforceable or invalid, that provision will be limited or eliminated to the minimum extent necessary, and the remaining provisions will remain in full force and effect.
Entire Agreement
These Terms, together with our Privacy Policy and HIPAA Notice, constitute the entire agreement between you and GLOBAL MED regarding the use of our services.
